INITIATING USE OF DEA CONTROLLED SUBSTANCES
AT MMRI

Step 1. Obtain and read MMRI Policy for the Use of DEA Controlled Substance. Sign and return the acknowledgement form found on the last page to Kayla Vuong, MMRI Regulatory Coordinator.

Step 2. Principal Investigator must fill out the PI request for Use of Controlled Substance and return to Kayla Vuong.

Step 3. Fill out the Controlled Substance Request for Personnel Information and return it to Kayla Vuong

Step 4. To order DEA controlled substance, fill out the Request Form and return to Kayla Vuong. ONLY MMRI STAFF AUTHORIZED TO ORDER CONTROLLED DRUGS MAY PLACE THE ORDER.

Step 5. You will be notified when your order arrives. The material will have been logged in and stored in the MMRI safe. Please note that currently, MMRI has only room temperature storage capability. Only authorized MMRI staff can dispense the drug to you.


Notes:

1. For Schedule II drugs, the drug MUST be listed on the MMRI DEA Research License. If not, it will take 2-4 weeks to add your drug to the license. See Kayla Vuong for details.

2. MMRI is authorized to ship controlled substance from MMRI to outside labs.
