Useful forms and Information:

MMRI Checklist for Signed Documents - provides a checklist of documents that must be read, signed and turned in to MMRI offices before initiation of certain activities at MMRI.

Lab Safety Training - OSHA requires that all lab workers be trained in laboratory procedures, personal protection, hazard communication and safety. Each affiliate company is responsible for ensuring that their employees are trained on an annual basis. MMRI offers annual training seminars and training resources. Affiliate Study Approval Sheet
- must be completed when requesting MMRI research services

MMRI Work Request - used for request for facility repairs. Fill out the form, put a copy each in Ron Snowden and Kayla Vuong's mailbox. Tape a copy onto the item to be repaired.

MMRI Materials Order Request
- used for ordering items through MMRI offices. MMRI will order for new affiliate companies for the first two weeks after joining the Institute. After two weeks, a 15% service charge will be added to your invoice. Turn in completed copy to MMRI personnel.

MMRI Intern Policy - Policy for summer interns at MMRI

Chemical labels - All chemical containers are required to have a label with the complete chemical name and the hazards associated with the material. If you transfer chemicals from a large container to a smaller one, you must label the new container with the full name and list the hazards. To make this job easier, the EH&S office has made some labels that you can print.
